About the sentence

to fill sth out

If you fill out a document, a form, a questionnaire or something similar, you add the requested/required information. e.g. We would appreciate it if you could fill out this short questionnaire about your stay at the hotel.

We can also use the phrasal verb 'to fill sth in' with the same meaning, e.g. If you want to make an official complaint then you will have to fill in this form.

Yes, I remember when I had to fill out several forms for a job I applied for. They were quite long and time-consuming. I am amazed at how much paper is wasted for this process, something has to be done so that this process won’t have an impact on the Earth.

Honestly, I’ve never filled out a form inaccurately nor have lied about anything, and I’d never do it either.
